Phoenix Services is about new life opportunities and new hope, supporting men and women with brain injuries and other similar disabilities to stretch to vastly higher levels of dreaming and achievement.

Mission: We empower people with brain injuries and others who need customized, unique and individual supports to maximize their potential by being part of their community and achieving personal goals.

Services Provided:

Supported Living: Provides one-on-one support, supervision, training and assistance for people to live as independently as possible. This service is available to those who live alone in their own homes, with roommates, a spouse or for adults who live with their parents.

Resource Facilitation: free, two-year support program that provides education and connection to supports and services to assist people throughout Utah in navigating life after brain injury.

Employment Services: Depending on the needs of the individual, a variety of employment supports are available. We begin with a thorough assessment to determine the job seeker’s abilities and interests. We then train them on the skills necessary to obtain and maintain employment in their field of interest. We also contact employers on the person’s behalf to find or create openings and arrange interviews. After placement, we assist the person to learn job requirements and adjust to the new workplace.

Host Homes: Host Home Support (HHS) is a great alternative to placing individuals in a group home. Individuals who choose to live in a Host Home will be placed in a home-like setting in a certified home with a trained family that offers support, supervision, training and assistance for adults.
